This site reads paperwork one clause at a time, so the most useful thing to write here is the clause that stopped you mid-read. Put it in the form together with the sentence before it and the sentence after it, so the clause is read in its setting. If it carries a price, a day count or a percentage, leave that figure in.
Claims about injectables, compounded products, or cell- and tissue-based products have published federal agency material to read beside them: a federal agency's page on safe injection practices, the FDA's page of compounding risk alerts, and the patient and consumer warning on unapproved products made from human cells or tissues.
If your document mentions botulinum toxin injections, a federal agency's page on harmful reactions linked to counterfeit Botox or mishandled botulinum toxin injections covers what went wrong in reported cases.
